Keeps Intake Manager apprised of status of outreach program, discusses problems and suggests changes to outreach activities. Carries out outreach program to market CCM to potential referral sources, and to maximize appropriate referrals to CCM. Assures timely nursing responses to referrals. Responsible for assessing eligibility and safety of patient in the home setting. Collaborates with other members of the Intake team in decision-making regarding patient's eligibility for program and formulation of plan of care.
