Case Manager - Point of Entry Department: Case Management Schedule: Full-time Shift: Day shift Hours: 10:00 am to 6:00 pm; Weekend Rotation Job Details: * Bachelors degree is preferred * Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write and communicate in English. Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational and interpersonal skills. Current New York State LIcensure as a Registered Professional Nurse or New York State Licensure to practice Social Work in the State of New York. Certification in Case Management required by 3/31/2013. PRI certification preferred.
