Nutritionist Department: CLHC-SELECT-QUEENS MLTC Schedule: Per Visit Shift: Day Hours: NO LUNCH - NO LNCH Job Details: * Evaluates and provides ongoing assessment of patient''s Nutritional Therapy needs, reports findings / patients'' status to the Community Health Nurse. Carries out the prescription of the physician for the individual patient in thePACECenteror at home; prescriptions may include (1) diet evaluation, (2) diet diary, (3) diet review. Participates in interdisciplinary team meetings to discuss patient''s progress in developing patient''s plan of care. Establishes frequency of visits based on physician''s order and in collaboration with the Community Health Nurse, documents patient''s progress in the medical record at each patient contact and completes appropriate program summaries. In-services staff and families/caregivers in treatment program. Assists in Utilization Review and Performance Improvement activities as requested. Candidate must have Bachelors Degree from an accredited Dietetics program along with New York State Licensed as a Registered Dietician and at least one year experience as a Nutritional Therapist, preferably in a Community or Outpatient settings. Home Care experience preferred. Bilingual English/Russian or English/Chinese required. Required to visit members in Queens and Staten Island.
