POSITION: Case Manager, Project LIFE
REPORTS TO: Executive Director of Community Services
CLASSIFICATION: Full Time
EXEMPT/NONEXEMPT: Exempt

GENERAL DESCRIPTION:
Provide case management services to individuals and families affected by Hurricane Sandy.
MAJOR DUTIES AND RESPONSIBLITIES:
- Conduct in-depth intake and interview with clients for needs assessment
- Maintain regular contact with individuals and families
- Document all contacts in progress notes
- Advocate for individuals and families with government and other social services offices to obtain assistance and relief
- Develop long-term recovery plan for each client
- Travel to Disaster Assistance Service Centers (DASCs) with client when needed
- Refer individuals and families for medical and mental health services as needed
- Develop and maintain relationships with outside disaster relief programs
- Ensure follow-up for all referrals and services
- Report any emergency situations and pertinent information to Director immediately
- Perform any other tasks assigned by Director and/or Supervisor

QUALIFICATIONS
EDUCATION:
Bachelor's degree in social work or related field or relatable experience required.
EXPERIENCE:
- 1 year casework experience preferred.
SKILLS & ABILITIES:
- Excellent written and verbal communication skills
- Respectful and comfortable with persons from different cultural and socio-economic backgrounds
- Ability to advocate for client effectively
- Knowledge of Microsoft Office applications
- Ability to navigate the internet
- Excellent time management, organization, and multi-tasking skills
