Founded in 1965, the mission of the Chinese-American Planning Council, Inc. (CPC) is to promote social and economic empowerment of Chinese American, immigrant and low-income communities.

Program Description:

Early Intervention Program (EIP) provides service coordination for children who have or are suspected of having a developmental delay and/or disability from birth to 3 years of age within the five boroughs of NYC. As much as possible, the Early Intervention services will be provided in your native language. Our Case Specialists speak the major Chinese dialects - Cantonese and Mandarin - as well as English and can help you with the services.

Position Title: Case Specialist
Location: 165 Eldridge St. New York, NY 10002, field visits is expected
Schedule: Full Time 35 hours per week, occasional weekends

Essential Functions and Responsibilities:
Case Specialist assists children 0 to 3 years old who have a disability or developmental delay obtain appropriate service. The Case Specialist will report to the Program Supervisor
Provide comprehensive service coordination to parents/caregivers of infants/toddlers aged birth to three under the Early Intervention (EI) Program.
Help families to negotiate the process of getting their developmentally delayed child evaluated and connected to therapeutic services.
Advocate and support parents/caregivers, translation, field visits, coordination and monitoring of services
Educate parents by informing them of their rights, options, and procedures undertaken in Early Intervention
Maintain caseloads and keep up-to-date progress notes with corresponding billing forms
Complete progress notes in a timely fashion and hold meetings with families and Early Intervention representatives
Any other duties assigned by Program Supervisor.

Qualifications and Skills
Bachelor’s Degree in Social Work, Counseling or related field preferred, with at least two years of relevant child welfare experience
Bilingual in Chinese and English a must.
Excellent written and oral communication skills, including interpersonal skills working with families and children
Computer literate
Flexibility in schedule preferred
