Housing Case Manager ($38,000) Provides case management services to veterans and their families that are: (a) person-centered; (b) collaborative; and (c) designed to help veteran/families to find, get, and keep permanent housing through advocacy, communication, education, social support, identification of service and support resources, and facilitated access to such resources. Qualifications: Bachelor's degree in a recognized social services discipline (social work, psychology, nursing, rehabilitation counseling, case management etc.) plus a minimum of 2 years' experience providing direct services to individuals experiencing homelessness or 3 years' experience working with people to prevent or end homelessness. Veterans and experience working with veterans is preferred.
