B-WEL CARE COORDINATOR

The Family Center
is a not-for-profit organization providing case management, supportive and legal services to women, youth and families affected by a myriad of challenges that threaten individual and family health and stability. The program Brooklyn Women Enjoying Life (B-WEL) supports Brooklyn women affected by substance use and HIV risk with the goal of reducing barriers to treatment, connecting women to treatment, and keeping women engaged in addiction treatment services.

RESPONSIBILITIES:
Engage and maintain relationships with staff members at partner agencies to ensure steady flow of clients into B-WEL program
Engage Brooklyn women who are completing treatment at a partner agency and ensure timely completion of B-WEL intakes, screenings, and assessments
Manage a caseload of B-WEL clients including providing counseling, case management, care coordination, referrals and follow-up
Accompany clients to various off-site appointments such as social services providers, medical visits, and public assistance and housing appointments
Advocate for clients in obtaining services, benefits and other entitlements and make home as needed to engage and retain clients in service and to complete data collection
Assist clients in developing a person-centered Recovery Action Plan describing specific goals and objectives for sustaining recovery​
Facilitate psycho-educational and support groups (Anger Management, Seeking Safety, SMART Recovery, WILLOW, and SISTA)
Conduct HIV and Hepatitis Rapid Testing, following agency protocol
Complete service delivery documentation in electronic health record
Provide internal and external referrals to callers not eligible for B-WEL services and to newly enrolled clients.
Assist with outreach activities in the community designed to raise awareness about B-WEL services
Attend weekly individual supervision with Project Supervisor and bi-weekly team meetings
Special projects as requested

MINIMUM QUALIFICATIONS:
Bachelor’s degree from an accredited college or university
CASAC, CASAC-T, CPP,CPS CARC, or CRPA preferred
Spanish bilingual preferred
HIV and/or Hepatitis testing experience, a plus
Two years social service experience with target population
Excellent phone and face-to-face engagement skills
Computer proficiency
