CLINICAL CASE COORDINATOR - SCO FAMILY OF SERVICES (BRIARWOOD/QUEENS, NY)
SCO IS LOOKING FOR AN EXPERIENCED CLINICAL CASE COORDINATOR FOR A QUEENS/BRIARWOOD LOCATION.
RESPONSIBILITES
Oversees placement and Crisis intervention when needed.
Must be present at all placements.
Participates in all conferences/meetings.
Available 24/7 cell phone coverage.
Provide recreational activities.
Provide transportation as needed.
Daily phone calls.
Complete consumer surveys.
Meets with youth and respite parent every 3-4 days and documents said contact.
Completes and collects all written paperwork in compliance with OMH.
Provides crisis management and stabilization as needed.
MINIMUM REQUIREMENTS:
Education/Degree: BA
Years of Experience: 3 – 5 years experience with SED population
MUST BE A TEAM PLAYER
MUST HAVE A VALID NYS DRIVERS LICENSE
