Please note that there is travel required utilizing personal vehicle or public transit for the position to local nursing facilities. This position is located in Harlem, NY.

POSITION SUMMARY
The Plan provides services to members residing in the community, requiring long term care services. The position utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes. Work at home considered after 3-6 months.



Required Skills:
EDUCATION
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS
Nursing/Advanced Practice Nurse is desired
Nursing/Certified Case Manager (CCM) is desired
Nursing/Registered Nurse (RN) is required

FUNCTIONAL EXPERIENCES
Functional - Clinical/Medical/Disease management/1-3 Years
Functional - Clinical/Medical/Quality management/1-3 Years
Functional - Clinical/Medical/Direct patient care (hospital, private practice)/1-3 Years

TECHNOLOGY EXPERIENCES
Technical - Desktop Tools/Microsoft Outlook/1-3 Years
Technical - Desktop Tools/Microsoft Word/4-6 Years
